Patient Portal

The Patient Portal is a secure, confidential way for patients to access their medical records. The portal enables you to request a referral, medication refills and to contact your medical provider and office staff with medical questions. You can enroll in the portal by registering with our office, with your email.

 Our office remains open during this time, but in order to reduce the risk for transmission, you may be asked several screening questions at the time of scheduling your appointment and again at arrival to your scheduled appointment. A facial Mask must be worn the entire time in our office, it must cover your mouth and nose properly, it cannot be under your nose.

How to protect yourself:

• Wash your hands often with soap and water for at least 20 seconds, especially after you have been in a public place, using the restroom, or after blowing your nose, coughing, or sneezing
• Use hand sanitizer that contains at least 60% alcohol, cover all surfaces of your hands, and rub them together until they feel dry
• Avoid touching your eyes, nose, and mouth
• Avoid close contact with people who are sick
• Social distancing

How to protect others:

• Stay home if you are sick
• Cover coughs and sneezes 
• Clean and disinfect frequently touches surfaces
• Practice good health habits
• Clean and disinfect frequently touched surfaces at home, work or school
• Get plenty of sleep, be physically active and manage your stress
• Drink plenty of fluids, and eat nutritious food

Insurance Referrals

We request 48 hours’ notice for non- urgent referrals.

All referrals are created and transmitted electronically.

Insurance

Please bring a copy of your insurance information with you, at the time of your visit. Co-payments are due at time of visit. 

We accept most insurances, if you are a new patient with an HMO plan we ask you to contact member services to appoint us as your primary care provider prior to your new patient visit. 

We mail patient statements on a monthly basis, we accept cash, check and credit cards, for any coinsurance, deductibles and any non-coverage fees. 

Fee Policies 

If you cannot keep an appointment, you are responsible for notifying the office a minimum of 12 hours in advance.  

Fee for missed appointment without minimum 12 hours notification:  $25.00

Forms-Completion of health forms requiring physician signature: $10.00-25.00 (fee based on form complexity)
